Our Story

Helping people stay home, not just stay safe.

A Guardians Hand LLC is a New York State–approved Home and Community-Based Services (HCBS) provider serving individuals enrolled in the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver programs.

Our mission is to empower individuals with disabilities and chronic health conditions to live safely and independently in their own homes and communities — rather than in nursing facilities. We provide person-centered services that promote independence, dignity, and community inclusion, with every service individualized around each participant's own goals, strengths, and needs.

We proudly serve eligible participants throughout the Lower and Upper Hudson Valley and New York City, helping individuals achieve greater independence, confidence, and quality of life.
